Mrs. O'Connell has this look. You know the one. Like she's already seen your chart, already decided what's wrong with you, and the cure involves her tits practically spilling out of that white coat. The game doesn't waste time with setup. You're Mr. Johnson, you hate checkups, but suddenly you're in the exam room and your cock has a different opinion entirely.

The assistant, Ellie, she's the one who really got me. Nurse Joy vibes but with less innocence and more "let me help the doctor with this... procedure." There's a moment where she's just watching, then she's not just watching anymore. The handjob starts clinical, almost proper, then the gloves come off. Literally. Mrs. O'Connell directs the whole thing like a conductor who's very interested in a specific kind of crescendo. The threesome doesn't feel like a porn scenario that just happens. It feels like these two have done this before. A lot. The way Ellie knows exactly when to move from hands to mouth, the way the doctor positions herself. It's a well oiled machine and Mr. Johnson is the lucky, terrified piston.

I wish the cumshot had more... geography. It lands, it's fine, but I wanted it to be a mess. I wanted it to be a problem they have to deal with. The game is unpolished on purpose, that's the bit, but the animation stutters in a way that pulls me out right when things should be at peak messiness. One frame Ellie's mouth is full, the next it's like nothing happened. Where's the aftermath? Give me a DLC that's just the cleanup. Ten minutes of these two dealing with the consequences, still in uniform, still professional but now slightly annoyed. That's the real fantasy.
